Fat Transfer to Face

Fat Injection has shown a rapid rise in aesthetic surgery applications in recent years. The reason for this is both the ease of application and the success of long-term results.

Fat injection;

1) to provide volume,

2) to reduce stiffness in previously degreased areas

3) to improve skin quality in areas with reduced skin quality

It is a method of injecting fat from any part of the body to another part.

The target of fat injections can be any part of the body.

Fat injection can be applied to areas with volume deficiency after an accident or congenitally, to areas with collapse after previous liposuction, as well as to plump the breast and/or hips. Fat injection is also used to provide volume and rejuvenate the back of the hand, where the lack of subcutaneous fat is particularly evident with aging.

However, the target of fat injection is often the face area. It is aimed to thicken the lips on the face, reduce the nasolabial lines, provide fullness to the cheekbones, create a forehead bump, reduce the under-eye pits and lengthen the chin tip.

Usually, when excess fat is removed (with 3D laserlipolysis), it is done in the form of giving the removed fat to areas with volume loss or where more volume is desired. After the application, the body contour is made more prominent and smooth.

The areas where fat is removed for fat transfer are usually the abdomen, waist or inside of the legs or inside the knees.

The amount of fat to be taken for fat injection can be quite variable. While 1000 cc of fat is used for hip enlargement, 500 cc may be sufficient for breast enlargement. In oil injections to the face, 5-60 cc of oil can be used depending on the application to be made.

Depending on the amount of oil to be used, it may be necessary to apply with local or general anesthesia. If a small amount of oil is to be used, the application can be done under local anesthesia in 30 minutes. However, if the amount of oil to be used is high, the time increases to 1-2 hours and the application should be done under general anesthesia.

Like all plastic surgeons, I pay attention to some things to increase the success of my fat injection. When taking fat, I use atraumatic cannulas that do not damage fat and tissues. I also take utmost care that the oil does not come into contact with the air and does not dry out. I use a system that draws oils with low pressure and collects them in special collector containers.

There are 3 types of fat transfer according to the size of the fat grafts: 1) Macrofat 2) Microfat 3) Nanofat fat graft

The first two are used to add volume, while the last one is used to improve skin quality, not to add volume.

The “Brazilian Buttock” (BBL-Brazilian ButtLift), which has become a trend in recent years, is made with fat injection. In BBL, excess fat is removed from the abdomen, waist, back and hip areas, and the waist is thinned and folds are created, and the hips are plumped and made more upright and prominent.

In breast augmentation, especially the upper part of the breast is filled, making the décolleté more voluminous and beautiful. In this type of fat transfer, the fat does not need to be thinned and the application is essentially Macrofat graft.

In the fat injections made to the face and hand region, first microfat and then nanofat graft definitions emerged. The use of macro fat grafts created problems, especially in areas with thin skin such as the face, and caused hardness and lumps. Therefore, its application in these regions was problematic.

The problems caused by the use of macrofat grafts on the face led to the development of Microfat graft application. In this application, the oil is separated from the connective tissues with special filters and its size is reduced. After the procedure, fats become able to pass through 1.2 mm cannulas without clogging and applying pressure.

Microfat graft is used in places where volume is required such as the back of the hand, nasolabial, forehead, chin and cheekbones. Micrograft fat injections create a volumetric lift effect when applied to a face with volume loss. Harmony is achieved in fullness and contour transitions. the skin becomes more vibrant and more beautiful.
